Auditor lifecycle framework

The Prepare, Registration, Operate, Verify, Enhance (PROVE) framework explains the key stages of becoming and remaining an approved auditor for the Heavy Vehicle Accreditation (HVA) scheme. It gives a clear, step-by-step overview of what is required—from preparing and applying, to operating as an auditor, being monitored, and continuing to improve. This helps make the process easy to understand, consistent, and transparent.

Prepare

The Prepare stage outlines the requirements to become a HVA scheme Approved Auditor and helps you get ready to apply. It explains the expectations, responsibilities, and standards required to perform audits under the scheme.

Eligibility/mandatory requirements

To become a HVA scheme Approved Auditor you must be an individual and meet the eligibility requirements in the National Audit Standard (NAS).

To be approved, you must:

  • Have the right qualifications and experience:
    • Complete a recognised Lead Auditor course aligned with ISO 19011 or equivalent (e.g. ISO 9001, ISO 45001 or integrated management systems).
    • Have relevant transport industry experience, or experience auditing safety systems in other industries.
  • Be properly insured:
    • Hold professional indemnity and public liability insurance.
    • Maintain this insurance at all times and provide proof each year.
  • Demonstrate capability:
    • Show you have the skills and knowledge to carry out heavy vehicle audits effectively.
    • Have strong communication skills.
    • Be able to work clearly and professionally with operators and stakeholders.
  • Maintain ongoing competency, professionalism and standards:
    • Take part in NHVR training or calibration activities when required.
    • Be considered a fit and proper person.
    • Follow ethical and professional expectations at all times.
    • Provide a criminal history check as required by the NHVR.
  • Declare conflicts of interest:
    • Disclose any potential, perceived or actual conflicts as required by the NHVR.

HVA scheme Approved Auditors will no longer be required to hold and maintain technical competencies, however the NHVR may introduce other training over time. Approval is valid for 3 years with ongoing training required to stay current.

Qualifications

Lead Auditors must hold the following nationally recognised units of competency from the BSB Business Services Training Package, or formally recognised equivalents.

These units may be obtained individually or through the Lead Auditor Skill Set (BSBSS00128).

Evidence requirements

  • Certificate or Statement of Attainment issued by an Australian Registered Training Organisation (RTO).
  • Unit codes clearly listed.
  • Nationally Recognised Training (NRT) logo displayed.

National qualifications provide a consistent and reliable standard for auditors in Australia. They ensure auditors can effectively plan, conduct, and report on audits across the full audit lifecycle.

Experience

You must have relevant transport industry experience or SMS auditing experience in another industry. We will assess whether your experience is suitable.

Insurance

You must hold professional indemnity and public liability insurance and provide evidence annually. Approved Auditors are responsible for ensuring their insurance adequately covers their audit activities and remains valid throughout their approval period.

Criminal history checks

You must complete a National Police History Check through an accredited provider. We will assess results as part of initial approval and ongoing monitoring to ensure auditors meet required professional standards.

Conflict of interest

Auditors must remain independent at all times. This is essential to preserve audit integrity, public confidence, and the credibility of NHVR accreditation schemes. Any actual, potential, or perceived conflict of interest must be declared to us as soon as it is identified.

Code of Conduct

You must follow the Auditor Code of Conduct in the National Audit Standard (NAS).

The Code outlines 11 key principles that guide auditor behaviour before, during, and after audits. You should understand these requirements before applying.

11 Core

Register

This stage explains how to apply to become an Approved Auditor, including the application process, training, and required checks.

Applications must be submitted by the individual applicant. You must meet all criteria and must not have previously had NHVR approval cancelled.

Application

To apply, complete the application form and provide evidence showing you meet all requirements.

  • Relevant audit or transport industry experience.
  • Proof of current insurance.
  • Criminal history check information.

Submit your completed application to the NHVR with all supporting documents. Incomplete applications may delay assessment.

Next steps

We will review your application. If successful, you will be invited to complete required online training and assessment through Litmos, our training platform. If further information is needed, we will get in touch to guide you through the next steps.

Litmos training and assessment

As part of the process, you’ll complete free online training through Litmos, including:

  • Auditor induction
  • Code of Conduct training
  • Safety Management System (SMS) training.

Once the training is complete, you’ll also need to complete an assessment. From time to time, additional training may be introduced to support consistency and keep auditors up to date with any changes. 

Approval

Once approved, you’ll receive your ID card and an official letter of approval. You’ll also be added to the register of Approved Auditors and can set up your dashboard to get started.

Operate

The Operate stage outlines what’s expected once you begin working as an Approved Auditor.

As an auditor, you’ll be responsible for planning and preparing audits carefully, considering the operator’s risk profile and relevant history. You’ll apply a structured approach to your audits, gather appropriate evidence, and ensure your findings are clear and well supported. You’ll use NHVR-approved tools and templates, maintain accurate records, and provide clear explanations of outcomes and next steps to operators.

Audit reports should be balanced, objective, and based on evidence, and submitted through NHVR Go. You’ll also be responsible for tracking non-conformances and ensuring corrective actions are followed through.

Throughout your approval period, it’s important to let us know about any changes that may affect your role.

Conducting audits

Audits are initiated by the operator. Once the auditor and audit date have been selected, the operator must submit an audit application to the NHVR at least 28 days before the scheduled date. The application must specify the audit date and location/s, the name of the auditor, and any other relevant details.

We will review the application before the audit can proceed. Once approved, you can begin preparing for the audit by reviewing the operator’s information and history.

Managing conflicts of interest

If you identify an actual or potential conflict of interest (COI), it’s important to pause and seek guidance. You should not proceed with the audit unless we have reviewed the situation and confirmed it’s appropriate to continue.

If a conflict arises during an audit, you’ll need to stop the audit and notify us as soon as possible. Being open and proactive about conflicts helps maintain trust and integrity in the audit process.

Audit quality

High-quality audits rely on clear, complete, and well-documented information. When using audit tools and completing reports, ensure all sections are filled in accurately and with enough detail so that another auditor—or the NHVR—can easily understand how findings were reached, without needing further clarification.

Maintaining HVA scheme registration

Your approval is valid for 3 years. To continue as an Approved Auditor, you’ll need to complete a skills assessment during renewal and continue building your knowledge and expertise. Meeting ongoing professional development and compliance requirements helps ensure consistent, high-quality auditing across the scheme.

Verify

The Verify stage focuses on supporting quality and consistency across audits.

We review audit outcomes and auditor performance to identify trends, provide feedback, and address any issues where needed. This helps maintain confidence in the scheme and supports auditors in delivering high-quality work. Monitoring may include audit reviews, peer or shadow audits, and performance insights.

Triggered audits and what NHVR monitors

We monitor key areas such as:

  • Audit report quality.
  • Use of evidence and risk-based approaches.
  • Conflicts of interest.
  • Auditor conduct.
  • Consistency across audits.

Reviews may be triggered by audit reports, incidents, complaints, or potential concerns around conduct.

Possible outcomes

Where needed, we may take steps to support improvement or address concerns. These may include:

  • providing feedback or guidance
  • recommending additional training
  • issuing formal warnings
  • temporarily suspending registration
  • cancelling registration in serious cases
  • requiring an audit to be redone.

The focus is on maintaining quality and supporting fair, consistent outcomes.

Assurance activities

To support audit quality, we carry out a range of assurance activities, including:

  • desktop reviews of audit documents
  • shadowing and skills assessments
  • spot checks of operators and records
  • reviews following complaints or incidents.

Where issues are identified, actions may include additional training, conditions on approval, or more serious measures if required.

NHVR rights

Maintaining the integrity of the scheme is essential. We may monitor and review auditor performance to ensure compliance with the NAS and relevant policies. This oversight helps maintain safety, reliability, and public confidence in heavy vehicle accreditation.

The Enhance stage supports your ongoing development as an auditor.

As an auditor, you’re expected to keep developing your knowledge and skills over time and meet any ongoing professional development requirements linked to your registration.
