A requirement of being an NHVAS accredited operator, is that you must tell us if you have a notifiable occurrence within a specific timeframe. We gather this information and use it to inform the transport industry of any wide-spread safety related concerns.
What is a notifiable occurrence?
A notifiable occurrence is a significant or major accident or incident that involves a nominated vehicle or driver operating under an Advance Fatigue Management (AFM) or Basic Fatigue Management (BFM) that has, or could have, caused significant property damage, serious injury or death and may include any of the following:
- A significant or major accident or incident involving a nominated vehicle, or a driver operating under an AFM or BFM
- A fire in a nominated vehicle
- A medical emergency involving the driver of a nominated vehicle or a driver operating under an AFM or BFM.
Note: Workplace slips, trips & falls are not notifiable occurrences.
What are the timeframes for reporting notifiable occurrences?

What is the difference between significant and major?
A significant accident or incident involves:
- injuries requiring hospitalisation and/or
- damage to property which presents a significant risk that requires considerable effort to avoid further impact or damage during recovery and clean up
- A major accident or incident involves:
- a fatality and/or
- damage to property with a major impact on life, property or the environment; generally accompanied by a high-level emergency response and clean-up and recovery operation.
How is a notifiable occurrence reported?
Notifiable occurrences must be reported in NHVR Go as an ‘incident report’ (via the ‘Information Hub’). Only details relating to the nominated vehicle(s) or driver(s) operating under AFM or BFM must be included. Do not include personal details relating to drivers.
Go through the questions and select your answers from the pre-written responses to form the body of the report. Enter relevant vehicle registration numbers when prompted. The system will check to see if the registration is in the NHVAS database. If the registration cannot be matched, you will be required to enter the vehicle details.
